Dental Insurance Benefits |
If you are insured please test:
Be familiar with the coverage and deductible on your insurance plans. If possible call your employer, human resource department or insurance company regarding any questions prior to your visit. Bring your insurance card or form with you.
Be aware that parents of minor children are responsible for any payment due. Non-emergency treatment for unaccompanied minors will be denied in the absence of a pre-authorized charge or failure to make previous arrangements with the financial coordinator.
Insurance Obligations
We accept Delta Premier and PPO, Aetna, United Concordia, Guardian, and Met Life, insurance plans. Co-payments are due at the time of treatment. As a courtesy we will file your insurance claim forms and answer any questions you have to the best of our ability. Deductibles, monetary limitations and payments for dental insurance vary widely from those you may be accustomed to with your medical provider. We will aid in securing the maximum allowance due to you. If insurance has not paid within 45 days of your treatment, you are responsible for making full payment to our practice and will be reimbursed when your insurance company pays Gentle Touch Dental Spa. All current documentation will be provided to you in order to assist in your inquiries.
Please keep in mind that dental insurance is an agreement between you, your insurance company and the employer who designed the policy.
